Abstract: in various health, safety, treatment, etc., the most frequent accidents are seen in the health and treatment sector. By identifying the risk factors that cause work-related accidents, it can be prevented. The purpose of this study is to evaluate the risk factors of accidents and priority Identifying risks and crisis classification It is for personnel to prevent it from happening.the failure analysis method ( FMEA:Failure Mode Effects Analysis ) was used to evaluate the existing risks. Kidney The work units of the company Shafa Teb Aso Kurdistan includes 5 stations Various work with 35 workers were evaluated and failure modes, failure effects and failure causes of errors were calculated. are Then the priority number RPN: Risk Priority Number (Risk Priority Number) for each calculation workstation and its priority crisis levels boxed are Found The results of this research have shown that 21 different risk factors have been identified among 5 different workstations In this company, 25 % of them are in the acceptable range, 55% of the risks are unacceptable and 20 % of the risks are at the emergency level. The highest risk is related to occupational therapy for clients due to exposure to various pathogenic agents dangers It is needle stickResult: There is a direct relationship between risk classification (RPN) and the frequency of accidents caused by work. Due to the high risks of work duties , the possibility of injury There are occupational accidents for company personnel. Despite the applicability of the FMEA technique in determining the risk factors of accidents caused by work, it is suggested to use the complementary technique of preliminary hazard assessment ( PHA: Process Hazard Analysis) should also be used for a more accurate and complete evaluation of this process.
